To ease the scheduling process, clients are encouraged to view clinic schedule and book their own appointments online.

  • The prices listed do not apply to Medicare recipients. Please see Medicare Policy​ for details.

We ask ALL clients (including Medicare recipients) to provide the credit or debit card information to reserve appointment.

  • We do not charge your card at the time of booking.

You can reschedule or cancel your appointment online at least 24 hrs in advance without penalty.

  • 50% (fifty percent) of the visit price will be charged to your card in case of missed appointments (no-shows), late cancellations, or late arrivals.

Your card will be automatically charged the full amount of the listed visit price one hour prior to your appointment.

Financial policy

With the exception of Medicare, Dr. Piker is considered an “out-of-network provider” for the commercial and state-sponsored insurance plans.

  • We do not charge your card at the time of booking.

If commercially insured clients wish to be reimbursed for visits with Dr. Piker, they may request the so-called “superbill” containing information needed for reimbursement from insurance carrier. The average reimbursement rate is around 60%.

Please keep in mind that the PPO plans are more likely to reimburse the services of an out-of-network provider than the HMO plans.

It is highly recommended that clients consult their insurance company regarding coverage of out-of-network providers prior to scheduling appointment.

The clients bear sole responsibility for knowing the specific rules of their insurance carrier.

Medicare policy

Dr. Piker is participating provider with the Medicare and will file claims with traditional Medicare insurance. Dr. Piker does not file claims with Medigap or other supplementary insurances. 

The prices listed on the booking page do not apply to Medicare recipients. ​The actual amount to be submitted to Medicare will be determined on the day of the visit.

Since Medicare covers 80% of the claim, the Medicare recipients will be responsible for the remaining 20% payable at the end of the visit. ​

We ask ALL clients (including Medicare recipients) to provide the credit or debit card information to reserve appointment. 

​Medicare recipients will also be required to sign the Advance Beneficiary Notice of Noncoverage (ABN) form prior to procedures informing them of the estimated costs in case Medicare does not cover a particular procedure.​

Cancellation and No-Show policy

You can reschedule or cancel your appointment online at least 24 hrs in advance by visiting the secure Client Area in your appointment confirmation email (Access Code will be emailed to you automatically).

We understand that life happens and, depending on the circumstances, it may be necessary to cancel or reschedule an appointment with less than 24 hrs notice. 

If it is less than 24 hrs before appointment start, requests to cancel or reschedule appointments should be done calling our clinic at 425-655-0700 or emailing us at [email protected]  

We do not double-book or stack appointments as our clients deserve undivided attention and adequate time during the visit. When someone cancels with little or no notice or simply does not show up for their appointment, that time is wasted and there is no one to fill the empty slot. 

Therefore, we implemented the following policy regarding last-minute cancellations and no-shows: 

  • Late cancellation is defined as cancellation or rescheduling with less than 24 hrs notice.
  • If you arrive more than 15 min late for 30 min appointment or more than 30 min late for 90 min appointment, you may be considered “No-Show”.
  • 50% (fifty percent) of the visit price will be charged to your card in case of missed appointments (no-shows), late cancellations, or late arrivals.

The clients are financially liable for charges incurred for late cancellations, late arrivals, or missed appointments as these are not covered by either private or government insurances.
